| A pro
forma is a standardised form
where a series of items have been assembled which are meant to gather a
coherent data set to support decision making. Each item has a question or
explanatory notes to prompt the user as to what information is required.
A pro forma may have optional items. Since
these items are not applicable to all cases, the person filling in the form
skips those items.
Frequently a pro forma will be used as the
basis of an assessment to determine what action to take.
Pro formas are used to:
- Standardise operating practice
- Ensure that a comprehensive set of
responses are provided relating to a particular case
- Ensure outcomes are well informed.
Checklists, surveys, questionnaires and
standard form evaluations are all pro formas.
Computerising pro formas can take place at
three levels:
- Making a ‘soft’ copy of the
original paper based form
- Incorporating sequence control to skip
optional or inapplicable items
- Using the computer to make the process
interactive with outcomes provided to the user responses.
Likewize pro forma is directed to all three
of these levels, with an emphasis on the last two.

There are three editions of Likewize pro
forma:
Browser Edition
This is used to run applications build using
Likewize pro forma where there is no requirement to build or change
applications.
Standard Edition
This edition is used to run applications, and
also allows the user to build or change applications which are of a modest
size.
Professional Edition
This edition is used to develop
applications for other people to use, and where the personal use results in
systems which are a larger and more complex than would be appropriate for the
standard edition.
